Screening for Gestational Diabetes

Testing for gestational diabetes is a controversial topic. There are several approaches and none are perfect. When I worked in gestational diabetes public policy with the state of California and in clinical practice, I saw firsthand the complexity of this issue. We know that consistently elevated blood sugar is risky for both mom and baby, but some screening methods for gestational diabetes run the risk of “false positives” or “false negatives.”

A lot of women don’t want to do the traditional glucose tolerance test, where you drink a super sweet “glucola” and measure your blood sugar response. If you identify with this, I don’t blame you. The drink tastes gross, it contains an ungodly amount of sugar, and it’s often loaded with preservatives and food dyes. That said, it’s how most research has differentiated between normal and abnormal blood sugar levels in pregnancy and remains widely used to this day. It’s not that it’s a bad test; it’s just not the best test for every woman.

Luckily, the research on gestational diabetes has come a long way in the last 20 years. This section will address the various testing options and the pros and cons of each.
